Indications/Uses
Management of type 2 DM (NIDDM) when hyperglycemia cannot be managed w/ diet & exercise alone.
Dosage/Direction for Use
Initially 1 tab once daily. Max: 1,500-2,000 mg once daily.
Administration
Should be taken with food: Take w/ evening meal. Swallow whole w/ full glass of water. Do not chew/crush/break.
Contraindications
Hypersensitivity. Renal disease or renal dysfunction, abnormal CrCl; acute or chronic metabolic acidosis w/ or w/o coma (including diabetic ketoacidosis).
Special Precautions
Renal impairment. Lactic acidosis. Discontinue in clinical situations predisposing hypoxemia, including eg, CV collapse, resp failure, acute MI, acute CHF & septicemia; temporarily discontinue prior to or at the time of intravascular administration of iodinated contrast media. Patients w/ CHF particularly unstable or acute CHF. Withhold in patients w/ dehydration &/or prerenal azotemia & 48 hr after radiologic study. Suspend therapy for any surgical procedure. Consider to discontinue therapy if exposed to stress (fever, trauma, infection, surgery) & administer insulin instead. Avoid excessive ethanol use. Increased CV mortality is possible. Elderly ≥80 yr. Pregnancy & lactation.
Adverse Reactions
Diarrhea, nausea/vomiting, flatulence; weakness. Chest discomfort, flushing, palpitation; headache, chills, dizziness, lightheadedness; rash; hypoglycemia; indigestion, abdominal discomfort, abdominal distention, abnormal stools, constipation, dyspepsia/heartburn, taste disorder; myalgia, URTI; decreased vit B12 levels, increased diaphoresis, flu-like syndrome, nail disorder; lactic acidosis, leukocytoclastic vasculitis, megaloblastic anemia, pneumonitis.
Drug Interactions
Increased effect/toxicity w/ cephalexin, cimetidine, iodinated contrast agents & pegvisomant. Decreased effect w/ corticosteroids, LH-releasing hormone analogs, somatropin & thiazide drugs. Hypoglycemia may occur w/ ethanol, chromium, garlic, gymnema. May decrease absorption of vit B12 &/or folic acid.
